Education Verification

To request an education verification, please fax a signed release to 815-588-8359. Please allow 2 to 4 business days to process your request.

Update Household Information

Change of Phone Number or Email: If you have changed your phone number or email, updates can be made through your Campus Parent Portal.

Change of Address: If you changed your address, email your proofs of residency to the Registrar's Office. 

Change of Custody or Guardianship: If there is a change in custody or guardianship, email your documentation to the Registrar's Office.
