Proofs of Residency

  • Three (3) Proofs of Residency Required Annually

The legal guardian of the student must provide three (3) proofs of residency prior to starting each school year.  All documents must be current (within the past 30 days) and include parent/guardian name with your District 205 address. A student’s address is the same as the person who has legal custody of the student.  To guard your security, please block out account numbers and social security numbers on the documents before you upload a copy of them.
