Enrollment Requirements

Freshman Enrollment
Please contact the Central Campus Registrar, Vivian Crose for a Registration appointment at 815.588.8251. You are required to submit the following forms at your Registration appointment:

Certified Birth Certificate
It must be the official birth certificate issued from the county/country of the child’s birth, with a county stamp or raised seal.  No hospital or baptism certificates will be accepted.  All foreign birth certificates are required to be accompanied by an English translation.

Copies of two proofs of residency
Two of the following, in the legal guardian’s name at the residing address:
- Tax bill
- Current utility statements
- Illinois Driver’s License along with proof of auto insurance   

Proof of guardianship
If you are the custodial parent as the result of court action, you must bring with you your divorce decree/custodial court papers showing demographic information that specifically states which parent has residential custody, and the last page bearing a judge’s seal, signature and date.  Additional documents may be required as deemed necessary by LTHS.

Freshman Physical Form

All incoming freshman must have a new high school physical and shot record completed on the DHS form.  Sports physicals are not acceptable.
